Alumni Transcript Requests

Graduates within the last 5 years can request a copy of their transcripts by filling out this form.

Please allow 7-10 days for us to process your request.

There is an $8 fee per transcript. You will be required to upload a valid driver's license along with proof of payment when completing your request. We are currently only accepting ONLINE payments at the moment. Please use the Revtrak link to process your payment.

You will receive an email once your Official Transcript is ready for pick up!

All other Alumni request past the 5 years will need to contact Student Records Services.

Current Student Transcripts & Records Requests

If you are a student currently attending Booker T. Washington HSPVA, you may request the following records by filling out the Student Records Request Form:

• Attendance Report
• VOE
• Report Card
• Official Transcript (The Attendance Office will email you when they are available for collection)
• Records for Social Security (Current Students)
• Records for Child Support (Current Students)
